1. Please complete the NEW PATIENT INTAKE here. You will need to upload a copy of your ID and any insurance card. All documentation is paperless for your safety and convenience.
2. Prepayment or insurance verification is required prior to reserving your appointment. New visits are $230 and followup visits are $210.  If you will be paying out of pocket you can make your  payment securely online by clicking on Office Visit “Pay Now” here: careclinicmd.com/fee/ or you can call us to pay by phone at 844-782-6963. If you are seeking to use your insurance benefits please call us to verify your eligibility. Insurance acceptance varies from state to state. You can confirm that we are in in-network with your plan here: careclinicmd.com/insurance . If we are out-of-network with your insurance, please check with your plan as you may be eligible for visit reimbursement.
3. Once received and a new patient slot is verified you will be contacted for an appointment within 24hrs or your requested appointment date and time will be confirmed. You can request your preferred date and time on our website here. If you need an earlier appointment, please call us for same-day availability.

New patient appointment availability is based on clinic capacity, and is first come-first serve. If you have a time sensitive matter please complete above paperwork as soon as possible to ensure a new patient opening.
