The weeks after a psychiatric admission, an ER visit or detox are when care matters most and is hardest to find. We pick up where the hospital left off.

What we do

Continuity, not a restart

Prioritized post-discharge appointments

Tell us you were just discharged and we move you to the front of the line for a first visit, by video or in person.

Medication reconciliation

We go through your discharge medication list line by line against what you were taking before, so nothing is doubled, dropped or left unexplained.

Coordination with your discharging team

With your consent, we request the discharge summary and speak with the hospital, ER or program that treated you, so your story does not restart from zero.

A plan for the first weeks

Follow-ups are closer together while things are fragile, then spaced out as you stabilize. You know when the next visit is before you leave the current one.

A crisis plan you and your family understand

Warning signs, who to call and what to do, written down and shared with the people you choose.

What to bring

Your discharge summary, your medication list and the name of your discharging clinician. If you do not have them, we will help you get them.

Anyone leaving a psychiatric admission, an emergency room visit for a mental health crisis, or a detox or residential program, who needs outpatient psychiatric care to continue.

I do not have a discharge summary yet. Can I still book?

Yes. Book the visit and tell us where you were treated; with your consent we request the records ourselves.

Will you talk to my family?

If you want us to. You decide who is part of your crisis plan and how much they hear.
